On November 18, 2025, union organizations Anaao-Assomed, Federazione Cimo-Fesmed, Aaroi-Emac, Fp-Cgil, Fvm, Uil-Fpl, Federazione Cisl-Medici, Cosmed, Cida, Uil, Cisl, and Aran signed a proposed contract for all medical, health, veterinary, and health profession managers for the three-year period of 2022-2024. The recent contract introduces notable economic changes, including:
1. An increase in the minimum monthly wage, for 13 months, of 230.00 euros, as of January 1, 2024. This increase includes and absorbs previous economic advances for 2022 and 2023, bringing the annual salary to 50,005.77 euros.
2. The position allowance (with charges borne by the Fund for the remuneration of assignments) consists of a fixed and variable part. This remuneration is set according to the type of assignment, reaching 52,966.00 euros for the assignment of direction of complex structure, while the initial professional assignment is set at 30,891.00 euros. Non-exclusive working executives are entitled to 65% of the position remuneration.
3. The medical-veterinary specificity allowance is redetermined to 9,466.00 annual gross euros (inclusive of the thirteenth month) from December 31, 2024.
4. The health specificity allowance is redetermined to 1,614.46 annual gross euros, from December 31, 2024.
5. An allowance for the direction of a complex structure has been redetermined to 11,157.00 annual gross euros for 13 months, starting from January 1, 2024.
Regarding overtime work, it is only permitted in exceptional circumstances or in response to a service call for ready availability. The hourly rates for overtime from the month following the CCNL signing have been redetermined to 30.72 euros for day overtime, 34.73 euros for night or holiday overtime, and 40.07 euros for night-holiday overtime.
Finally, the agreement includes a guarantee clause for executives with exclusive relationships and a positive evaluation, for whom a minimum compensation value is ensured based on their years of service, even temporary. These minimum values are set at 6,798.00 euros for 5-15 years of service, 7,600.00 euros for 15-20 years of service, and 9,100.00 euros for service equal to or more than 20 years. If the total position salary of the assigned role is less than these values, it is increased until the minimum guaranteed threshold is reached.
